- Requirement:
• Each school has their own envelope with 12 (twelve) codes written on the outside of the envelope.
• The contractor will enter the dates in the format ‘mmddyyyy' and place a zero before single-digit months and days.
• For example, may 8, 2021 or 5/8/21 shall be entered as "05082021".
• Schools may use both sides when copying their immunization records.
• Contractor must double-check to make sure that records are not overlooked on the backside of another record.
• Schools may submit immunization records which are not written on the standard immunization record form.
• The information may still be extracted from these forms, so long as the birth date is available.
• Some immunization records may be incomplete (i.e., some dates for the vaccinations were not recorded).
• As long as the birth date is visible, the contractor shall enter the birth date and the dates that were recorded for that individual child.
• Type in "15" if vaccination is missing the day.
• If the month or year is missing, then the contractor shall enter no data.
• Occasionally vaccination dates may be missing, for example hib1, hib3, hib4 are complete but no data was entered in hib2.
• This case, the contractor shall enter hib3 as hib2 and then enter hib4 as hib3.
• The contractor must keep track of the order in which data was entered so that when data is to be reentered and verified, the contractor can easily reduplicate what was originally entered.
• Contractor shall assign each individual immunization record its own record number visible in the bottom right-hand corner of the immunization record.
• Case id number is to be written on the immunization record.
